Does medication treatment change one's personality?

No, when proper, medication does not change one's personality.

If, by personality, we mean emotional and behavioral traits that characterize an individual, then those properly treated with medication for their ADD/ADHD should notice only the loss of undesirable traits such as harmful impulses and urges.

No one, in my opinion, should take ADD/ADHD medication that does not "feel right" or in any way changes one's personality in an undesirable way. That does not happen to those who are taking the right medication at the right dosage.

I have heard complaints from a small number of treated ADD/ADHD people, usually teenagers who, for example, gave up being the class clown or a social butterfly, that medication has had a positive, constructive influence but an "unwanted" effect on their social status.

This kind of complaint tells us something we already know. Humans often hate giving up old behaviors even if they are wacky and somewhat harmful.
